Type and Scope of services to be performed:

 

Scope of work:

                     Colorado Health Network will provide Rental Assistance (TBRA), Short Term Rent Mortgage Utility Assistance, Supportive Services, and Permanent Housing Placement programs to moderate to low-income (80% AMI or less) individuals living with HIV/AIDS.

 

                     Service Program overview:

                     Tenant Based Rental Assistance (TBRA) - provides housing assistance to eligible households. TBRA meets the needs of participants by subsidizing the difference between total rent and the monthly tenant rent contribution.

 

                     Supportive Services - Collaboration with other case management services offered through Denver Colorado Aids Project such as referrals to healthcare, support around adherence to healthcare and treatment, referrals to mental health and substance abuse counseling, referrals to support groups and psycho-educational workshops, housing resources and referrals, and vocational assistance.

                     

                     The HOPE Program - provides supportive services to homeless HIV/AIDS persons including assistance with medication adherence and education, HIV specific nutrition and service referrals including links to transitional/permanent housing options, support groups and counseling, and emotional support.                     

 

                     Short Term Rent Mortgage Utility Assistance (STRMU) - provides housing assistance to eligible households for up to $2,000 and/or 21 weeks (continuous or non-continuous) of assistance in a 52-week period. The 52-week period for this program aligns with the calendar year. This program is designed to prevent homelessness by assisting to retain long-term, stable, permanent housing options for households that might otherwise lose their housing.

 

                     Permanent Housing Placement - provides deposit/move-in assistance to eligible participants.

 

Executive Summary

This contract was selected for Department of Housing Stability (HOST) funding as a result of the department’s Notice of Funding Availability (NOFA) procurement process.

 

HOST initiated a Notice of Funding Availability (NOFA) procurement process in August 2020, as a basis for awarding many of the department’s housing opportunity, housing stability and homelessness resolution programs/services contracts for 2021. HOST is now finalizing amendments to many of these contracts for the extension of services throughout 2022.

 

HOST sought proposals for a variety of programs and services utilizing both federal and local funds. The 2021 Housing Opportunity, Housing Stability, and Homelessness Resolution Programs NOFA specifically solicited proposals for programs or services to fund activities in the following areas:

                     Housing programs or services

                     Housing stability

                     Homelessness resolution

                     Innovative programs and services

                     Professional facilitation services


Federal funding sources identified under the NOFA include Community Development Block Grant Program, Home Investment Partnerships Program, Emergency Solutions Grants, Housing Opportunities for Persons with AIDS Program, and CARES Act resources to support coronavirus response. Local/other funding sources included General Fund, Affordable Housing Fund, foundations, and individual donors that support housing development, preservation, and programmatic assistance for low- and moderate-income residents.

 

This project will fund Rental Assistance (TBRA), Short Term Rent Mortgage Utility Assistance, Supportive Services, and Permanent Housing Placement programs to moderate to low-income (80% AMI or less) individuals living with HIV/AIDS.
