A resolution approving a proposed Second Amendatory Agreement between the City and County of Denver and The Colorado Coalition for the Homeless to provide continued programming for the Supportive Housing Pay for Performance program.
Amends a contract with Colorado Coalition for the Homeless (CCH) by adding $2,995,267 for a new total of $8,159,233.00 and adding one year for a new end date of 12-31-2022 to provide continued programming for the Supportive Housing Pay for Performance (formerly Social Impact Bond) program, citywide. (HOST-202261987). The last regularly scheduled Council meeting within the 30-day review period is on 5-23-22. The Committee approved filing this item at its meeting on 4-27-22.

Type and Scope of services to be performed: CCH will comply with the terms of the SHP4P Extension evaluation plan, which amends the City contract numbered FINAN-01523939, as such evaluation is amended under the terms of such City contract, including enrollment mechanisms, reporting, and participation in evaluation related activities. This includes providing an appropriate level of care based on clinical assessment, integrated with a flexible array of housing options delivered through an evidence-based Housing First approach to provide housing and supportive services for the costliest members of the “Super Utilizers” population. CCH will provide housing services, supportive services, case management, treatment options, and complete stipulated data reporting.

Provider was selected for the original social impact bond project and has supported the evaluation functions since inception. Continuation of CCH as the provider of housing and services provides continuity of service and data as the project continues. Similarly, it ensures continuity of housing for participants of the project.
