A bill for an ordinance approving a proposed Agreement between the City and County of Denver and Denver Health and Hospital Authority (DHHA), for maternal child health services for care coordination for children and youth with special healthcare needs.
Approves an intergovernmental agreement with Denver Health and Hospital Authority (DHHA) for $500,000 and through 09-30-2023 for maternal child health services for care coordination for children and youth with special healthcare needs, including general consultation to families, service navigation, coordination between providers, and other services, citywide (ENVHL-202265544). The last regularly scheduled Council meeting within the 30-day review period is on 1-17-2023. The Committee approved filing this item at its meeting on 11-30-2022.

Type and Scope of services to be performed:
Denver Health & Hospital Authority is subcontracted to execute work included under Strategy 1 on the Access to Supports tab of the Denver Department of Public Health & Environment Maternal Child Health Action Plan (attached), namely care coordination.
The Denver Health & Hospital Authority team has provided these services to Denver’s children and families for more than 30 years, and services this year will become even more critical as families deal with the effects and delays caused by the pandemic. Additionally, as of 12/31/22, Tri-County Health Department will be dissolved. Adams, Arapahoe and Douglas counties have not yet finalized their care coordination plans, so the likelihood of Denver’s team serving even more families and providers from these counties is high. Neighboring counties (El Paso, Jefferson, etc.) also plan to downsize their care coordination efforts in coming years, which could potentially increase the criticality of Denver’s program even more.

Objectives:
• By September 30, 2023, ensure that at least 127 families with children and youth with special health care needs receive patient and family-centered care coordination that integrates physical health, oral health, mental health, and community-based services.
• Complete and submit the Children and Youth with Special Health Care Needs Decision Tool (if not already fully completed in FY22).
• Continue to participate in Denver Health Autism Referral Group.
• Continue to recruit community stakeholders, including community partners, agencies and families in the autism and mental health communities.
• Conduct ongoing meetings, as needed, with community stakeholders to create a community process/plan to address the identified system changes needed for the autism evaluation process and access to services for children and youth with autism and a mental or behavioral health need.
• Pilot implementation of one activity identified for systems change and evaluate outcomes, and collect community and family input around outcome.
• Develop Action Plan for FY24, with a targeted focus on children/youth with autism and a mental or behavioral health need.

Sole- State and Federal requirements require that at least 50% of MCH funds are dedicated to children and youth with special healthcare needs. The DHHA HCP team, led by Molly Benkert, provides care coordination for Denver’s children and youth with special health care needs, and has done so for more than 30 years.
