Type and Scope of services to be performed:

A.                     Rehousing services will be provided to families participating in the Lambuth Family Center shelter programming. These rehousing services will include the following:

a.                     Housing-focused case management with other supportive programming activities

i.                     Case managers will utilize the Housing First Model

ii.                     Barriers will be assessed to address needs in obtaining and maintaining housing

iii.                     Case Managers will utilize the Coordinated Entry assessment to provide households with access to housing opportunities through the Continuum of Care

B.                     The Lambuth Family Center will provide 24-hour shelter operations to approximately 20 families at a time offering shelter and other necessities, including three meals a day, access to laundry facilities and clean linens.

C.                     The Lambuth Family Center will provide Housing support to families using case management to assist families with accomplishing goals, including securing financial security, and obtaining long term housing.

a.                     Households will be provided with weekly case management support and will be provided with shelter until they are able to transition into long term, permanent housing. Additionally, they will have access to life skills classes, money management, parenting supervision, education assessments and tutoring, GED preparation, job readiness classes, and financial classes

b.                     Children in the program will have access to one on one tutoring and educational advocacy within the school system
