A resolution approving a proposed Second Amendatory Agreement between the City and County of Denver and Family Promise of Greater Denver to continue providing, through HOST homelessness resolution program, temporary housing for families experiencing homelessness while they wait to move into already-identified permanent housing, citywide.
Amends a contract with Family Promise of Greater Denver by adding $225,616 for a new total of $902,464 and one year for a new end date of 12-31-2024 to continue providing, through HOST homelessness resolution program, temporary housing for families experiencing homelessness while they wait to move into already-identified permanent housing, cityside (HOST-202369741-02 / 202157506). Type and Scope of services to be performed:
A. Family Promise of Greater Denver will form partnerships with landlords and local hotels to master lease apartments and rent hotel rooms for Bridge Housing clients.
B. Weekly case management will be provided to families to support efforts toward increased income, acquiring of necessary documents, connection to mainstream benefits and long-term stability.
C. All households will have an identified housing resource such as a tenant-based or project-based housing voucher, market rate rental unit or a housing resource that has been matched through Coordinated Entry.
