Type and Scope of services to be performed:

Once the Housing Navigator is hired and the contracts with the housing navigation provider and sober livings are finalized, various levels of housing support will be available to releasing individuals based on their expressed needs, substance misuse and/or mental health diagnoses, history of homelessness, reentry plan, and length of stay in the jail. The final determination will be made by the individual’s DSD case manager and the Housing Navigator. These levels of support include:

 

                     Emergency housing support: this support will be provided to unhoused individuals who were in jail for only short periods of time, were not engaged in services long enough to access one of the other levels of housing support or were not eligible for or accepted to the other levels of housing support. Emergency housing support includes vouchers to shelters or motels for a period of approximately 1-14 nights.

                     Rental support: this support will be provided to individuals who were housed prior to their period of incarceration and need rental assistance to maintain their housing, or to individuals who are unhoused but have identified (either independently or with the support of the Housing Navigator) a rental property to move into after release. The standard period of support will be three months of rent (including fees), and any application fees or security deposit for individuals moving into a new rental property but may be less or more at the discretion of the Housing Navigator and case manager. but may be less or more at the discretion of the Housing Navigator and case manager.

                     Sober living support (transitional housing): this support will be provided to individuals with substance misuse disorders who are unhoused and interested in sober living. The standard period of support will be three months of rent, and the application fee or security deposit if required by the sober living provider but may be less or more at the discretion of the Housing Navigator and case manager.
