Caring for Denver Foundation

Type and Scope of services to be performed:

These funds will allow the city to contract with providers of behavioral health and substance use services to support people on their journey to permanent housing. Grant funds will be shared by HOST and DDPHE.

 

This grant will cover a portion of HOST’s contract with CCH to provide behavioral health services via their mobile unit at designated sites (HOST-202371124). Through this contract, CCH will provide mental and physical health care services and micro-community locations, and up to three hotel sites serving as non-congregate shelter. Services will include the following:

                     Leveraging both the Health Outreach Program (HOP) and its Street Medicine Team, CCH will visit at least one micro-community or hotel per business day while there are three or less micro-communities. When there are more than three micro-communities, CCH will visit a minimum of two sites, either hotel or micro-community each business day, as dictated by level of client need.

                     Services will generally be provided at micro-community sites between the hours of 8am-4pm, as determined by level of client need. Schedules may vary based on the size and unique needs of each micro-community.

                     Screen and assess clients to determine clients’ need for mental and physical health services.

                     Using appropriately credentialed staff, address clients’ mental and physical health needs providing a trauma-informed, harm reductionist, motivational approach to care.

                     Build rapport and create sustainable outcomes that drive clients to engage and follow treatment plans (i.e., Motivational Interviewing; Screening, brief intervention and referral, or provision of treatment as needed).

                     Provide onsite Medication-Assisted Treatment (MAT) utilizing Suboxone, in compliance with all Federal, state, and local regulations and laws, in combination with counseling and behavioral health therapies specifically supporting people with opioid use disorders (OUDs).

                     Provide psychiatric support for people living with psychiatric diagnoses including but not limited to depression, post-traumatic stress disorder, anxiety disorders, and other psychiatric conditions.

DDPHE will administer the remainder of funds ($1,148,040) to several agencies that can provide a menu of behavioral health treatment and support services. The primary focus of service delivery will be for those living at the sites and will include peer support, group and individual therapy, and creative or non-traditional modalities that support individual recovery goals. Grant funds may also be used to support the mental health needs of staff operating the sites, as well client needs related to accessing behavioral health support services (such as transportation).
