Amends a contract with the Colorado Coalition for the Homeless (202057227) to add $1,327,663 for a new total of $4,911,652 and 1 additional year for a new end term of 12-31-2024 to provide temporary housing and support to individuals and households experiencing homelessness who have an identified housing resource, citywide. 202370243-02. The last regularly scheduled Council meeting within the 30-day review period is on 12-4-2023. The Committee approved filing this item at its meeting on 11-1-2023.

Type and Scope of services to be performed:
A. The Bridge Program will serve highly vulnerable individuals and families experiencing literal or chronic homelessness, primarily referred through Coordinated Entry and community partners providing outreach services.
B. Bridge Program staff will work collaboratively with CCH service teams and outside partners to provide households with completion of applications for housing, outreach to landlords, searches for appropriate housing units and access to needed wraparound services such as medical, behavioral health, substance use, and other clinical treatment.
C. Once Bridge Program participants transition into their permanent supportive housing, they will be provided with ongoing case management, clinical counseling, and other supports to help them maintain housing stability.
