Amends the contract with Colorado Coalition for the Homeless due to a change of Exhibit C which is the revenue grant attachment. The revenue grant attachment was changed as a result of Federal grantor, U.S. Department of Housing and Urban Development (HUD), consolidating multiple Continuum of Care grants under the Housing First Program (SOCSV-2013-11408-01).
