Amends a contract with the Colorado Coalition for the Homeless to extend the term through June 30, 2013, and to increase the contract by $190,000 for a total contract amount of $570,000, to provide clinical support through substance treatment services, ongoing medication management, and case management to chronically homeless persons with severe and persistent mental illness (SOCSV-2012-08204-02).
