Amends an agreement with Colorado Coalition for the Homeless (CCH) by adding an additional year of services (a new end date of 12-31-15) at a cost of $144,000 (for a new total cumulative cost of $1,296,000) to provide Assertive Community Treatment “ACT” to mentally ill-homeless persons in the Court to Community court intervention program C(E-70053 (8)).
