Amends an intergovernmental agreement with Denver Health & Hospitals Authority to increase the amount by $349,999.00, for a total of $1,049,997.50, and to extend the term one year through 12-31-2016 to provide transitional residential treatment, case management and/or mental health services for up to 100 chronically homeless individuals (SOCSV2013-14440-02).
