Type and Scope of services to be performed:

This Amendment 04 is for a name change only.  The firm changed their name from Tsiouvaras Simmons Holderness, Inc. to RS&H, Inc. The original purpose of this agreement is as follows: The City seeks professional design engineering and related consulting services for the Reconstruction of Federal Boulevard from 5th Avenue to Howard Place in accordance with the Proposed Action as stated in the Planning Environmental Linkage Study (PEL) completed in October of 2009 for this stretch of Federal Boulevard.  Three southbound lanes and three northbound lanes will be constructed, as well as a raised center median.  Services are described in the RFP and will include public involvement, stakeholder and agency coordination, engineering and related design (traffic, traffic control, roadway, civil, geotechnical, landscaping, pavement, drainage, erosion control, hydrology and hydraulic, water quality, lighting, and construction phasing), utility coordination, surveying, right-of-way, and environmental services.  Cost estimating and scheduling will also be required.  Services during bidding will be required, and once the project is in the construction phase, the consultant will provide services during construction up to and including full construction management, inspection, and materials testing services
