A resolution approving a proposed Amendatory Agreement between the City and County of Denver and Savio House to provide intensive case management services for at risk families, citywide.
Amends a contract with Savio House by adding $198,800 for a new total of $764,445 and 11 months for a new end date of 8-31-21 to provide intensive case management services for at risk families, with the goal of keeping youth in their homes, preventing out of home placement, and effectively transitioning children from out-of-home placement back home, citywide (SOCSV-2017-37946-04; SOCSV-2020-56289-04). Authorizes a fourth amendment to the contract with Savio House for Denver Human Services (DHS) through contract control number SOCSV-2017-37946-04 & Jaggaer SOCSV-2019-56289-04to add $198,800 and 11 months, for a new end date of 8-31-21 and a new contract total of $764,445.
Savio House will provide intensive case management services for at risk families in Denver County, with the goal of keeping youth in their homes, preventing out of home placement, and effectively transitioning children from out-of-home placement back home. Crisis Intervention, direct client contact, resource development, and support services will be provided to mitigate safety concerns and secure safety in the home.
