Type and Scope of services to be performed:
Rehousing or Rapid Rehousing is a housing model that combines subsidies such as rent assistance with supportive services for a pre-determined period. These services can include housing navigation, employment assistance, peer support, and case management. Rehousing is a short-term intervention designed to produce independence afterward; ideally, the person exits the rehousing program not only having obtained housing, but also with employment or other income to sustain housing long-term.
This agreement aims to deploy resources that can support those who need a limited amount of assistance in order to exit homelessness through a return to stable housing. Colorado Coalition for the Homeless, as the lead agency, will work with provider partners to form a Rehousing Collaborative. Collectively, the Collaborative will provide short-term bridge shelter, rapid rehousing and rental assistance, and supportive services to qualifying participants who are experiencing homelessness.
