Amends a contract with the Colorado Coalition for the Homeless by adding an outcome related to households served for administering the Denver Street Outreach Collaborative (DSOC) and Strategic Outreach to Large Encampments (SOLE) programs. No change to contract amount or duration (HOST-202057228-01). The last regularly scheduled Council meeting within the 30-day review period is on 8-30-21. The Committee approved filing this item at its meeting on 8-4-21.

Type and Scope of services to be performed:
This project funds The Colorado Coalition for the Homeless’ Denver Street Outreach Collaborative (DSOC) and Strategic Outreach to Large Encampments (SOLE) program. DSOC and SOLE utilize a model of street outreach that relies upon consistent, long-term, trusting relationships between outreach workers, a case manager, a licensed behavioral health care provider, peer specialists, Registered Nurses and people, both adult and youth, experiencing unsheltered homelessness. This contract will make contact with 6500 individuals through the DSOC program and will serve these households via the SOLE program. This contract amendment will add the outcome related to households served, which was missing in the previous iteration of this contact. The previous contract did not identify this outcome benchmark. This contract will now serve 6500 duplicated households. No change to the contract amount or duration.
