Type and Scope of services to be performed:

1.                     Bridge to Home Program will serve highly vulnerable individuals and families experiencing ‘literal’ or ‘chronic’ homelessness, sleeping in places not meant for human habitation, such as streets, cars, and encampments.

2.                     The Bridge to Home Program will primarily serve households referred through OneHome and outreach to encampments but may also serve individuals and families staying at an emergency shelter or safe haven, or fleeing, or attempting to flee domestic violence, dating violence, sexual assault, stalking, and/or other dangerous or life threatening conditions related to violence, who have no identified subsequent residence; and lack the resources and support networks needed to obtain other permanent housing.

3.                     50% of referrals will come from OneHome and 50% referrals will come from the Denver Street Outreach Collaborative.

4.                     Bridge Housing units will include units at Fusion Studios, 3737 N. Quebec St.; Beacon Place, 3636 W Colfax Ave; master-leased apartments, and master-leased motel units.

 

5.                     Once Bridge housing participants transition into their permanent supportive housing, clientele will be provided ongoing case management, clinical counseling and other supports to help them maintain housing stability. These services will be provided by CCH's Housing First, Residential Services, Family Support Services, or other service providers.
