Type and Scope of services to be performed:

 

Requesting No Cost Extension with Denver Health & Hospital Authority for original contract amount of $7,985,700.00 and extending the contract for one year (through December 31, 2023). This amendment extends Denver Health and Hospital Authority (DHHA) assistance with the City and County of Denver’s Department of Public Health and Environment (DDPHE) by providing personnel, services, and supplies necessary to perform critical COVID-19 containment activities, such as disease surveillance, case investigation, contact tracing, and testing, based on jurisdictional needs. These services extend across seven distinct project services delivery areas of operation and include:

 

                     PHEP Funding (Project A)

                     PHEP COVID Supplemental (Project B)

                     ELC COVID (Project C)

                     Coronavirus Relief Fund - CRF (Project D)

                     PHEP Epidemiologists (Project E)

                     Citywide COVID SRF - Nurseline (Project F)

                     COVID On-Call Services (Project G)

Denver Health and Hospital Authority (DHHA) will assist the City and County of Denver’s Department of Public Health and Environment (DDPHE) by providing personnel, services, and supplies necessary to perform critical containment activities, such as disease surveillance, case investigation, contact tracing, and testing, based on jurisdictional needs.
