Type and Scope of services to be performed:

The purpose of this contract agreement is to provide Continuum of Care (CoC) Permanent Supportive Housing (PSH) for those who have disabilities. PSH is designed to assist individuals and families who are experiencing homelessness and provide them with services needed to help them move into permanent housing, with the goal of long-term stability. The Department of Housing Stability (HOST) will provide subaward funds in the amount of $545,740.00 to The Empowerment Program Inc. (“TEP”) to be utilized for the purpose of providing permanent supportive housing to 31 individuals participating in the CoC Bedrock Lowry program. All households will be referred to TEP through the Coordinated Entry System (OneHome). This contract will provide funding for rental subsidies, case management, and administrative costs.

 

This subaward is not for Research and Development. TEP is identified as a subrecipient for the purposes of this agreement and is therefore subject to all terms, conditions and regulatory requirement required of federal funding subrecipients per 2 CFR Part 200, as well as specific rules and regulations for Continuum of Care program.

Continuum of Care funding and providers are selected and approved through a similar competitive process, the HUD Continuum of Care Notice of Funding availability. Projects are scored and ranked by the Metro Denver Region’s Continuum of Care (Metro Denver Homeless Initiative) the year prior (2021 in this case). Awards are made the subsequent year (approved through a previously approved Grant Agreement, HOST-202264005).  A Sole Source document was used for the development of this contract.
