Amends a contract with the Colorado Coalition for the Homeless by adding $120,000 for a new total of $532,200 and one year for a new end date of 12-31-2023 to continue providing financial assistance for rent and other housing-related costs and to provide supportive services for at least 37 households to ensure housing stability as part of the Social Impact Partnerships to Pay for Results Act (SIPPRA), citywide (HOST-202261990-00/HOST202366381-01). Type and Scope of services to be performed:
The amendment provides additional funds to the Colorado Coalition for the Homeless to be used for rental assistance, supportive services, and other housing related expenses to ensure housing stability for 37 households enrolled in the Social Impact Partnerships to Pay for Results Act (SIPPRA) from the U.S. Department Treasure. Eligible households must be homeless, have a record of at least eight (8) arrests over the past three (3) years in Denver County, and be at high risk for avoidable and high-cost health services paid through Medicaid (including services received at Denver Health).
Scope of work:
A. To support permanent supportive housing, CCH will provide rental assistance and bridge housing for at least 37 households enrolled in SIPPRA.
1. SIPPRA participants must be homeless; have a record of at least eight arrests over the past three years in Denver County and be at high risk for avoidable and high-cost health services paid through Medicaid, including services received at Denver Health.
B. CCH will collaborate with City and County of Denver and other community partners to establish a list of eligible participants.
1. After an individual has been deemed eligible for the project by the Urban Institute and has consented to participation, they will be randomly assigned to receive SIPPRA rental assistance and services.
2. Individual will be referred to the Referral Coordinator with the Crime Prevention and Control Commission to CCH.
