Type and Scope of services to be performed:
Contractor will provide planning and navigation services to remove housing barriers for people with I/DD by supporting them in working within and beyond complex affordable housing systems towards housing stability. Contractor housing navigators will work with residents with I/DD who are experiencing homelessness, at-risk of displacement from current housing situation, and/or seeking a more independent living situation by supporting connectivity between the individual and various housing opportunities available in Denver. This program includes supports for planning with individuals who live with aging caregivers. Contract funds multiple FTE and provides resources which will support the implementation of this pilot program over the next 2 years and 8 months, with the current contract term concluding December 31, 2025.
