Type and Scope of services to be performed:

SERVICES DESCRIPTION

A. Shelter Operations

1. VOACB will provide 24/7 day and evening shelter support for up to 45

senior women experiencing homelessness, and women veterans experiencing  homelessness.

2. VOACB will maintain and offer on-site client access to sleeping, community,

and dining space, and a secure outdoor courtyard.

3. VOACB will maintain and offer off-site client access to VOACB emergency

services, including clothing bank, computer labs, and a meal program.

4. VOACB will provide clients with a clean bed and bedding, shower access,

laundry facility access, hygiene supplies, nutritious nourishment comprising at

least 2500 calories per day

B. Shelter Operations

1. VOACB will offer support services through a low demand, creative engagement

approach guided and adapted to individual needs of the guests.

2. VOACB case management will offer engagement to maximize exits into

permanent and stable housing.

i. Completion of HMIS assessments, including, OneHome coordinated

entry assessment.

ii. Assistance with completing applications to affordable, subsidized senior

housing options, including VOACB Affordable housing for seniors, and

screening for appropriate placement in VOACB Irving Street Women’s

Residence (HUD Safe Haven).

iii. Coordinating co-living arrangements with a focus on other senior women,

shared group living options, group home or other residency through

WellPower or other behavioral health providers.

iv. Reunification with family members or friends.

v. Assisting in the access of Denver area assisted living facilities when

necessary and appropriate, and other housing opportunities for which the

client qualifies. This may include working with the local Single-Entry

Point to access Long-Term Care Home & Community Based Services.

vi. Providing support in obtaining vital documents and benefits enrollment

(e.g., financial, transportation, employment assistance.)

vii. Making referrals to mental health support and substance-use support

assistance.
