A resolution approving and providing for the execution of a proposed amended Grant Agreement between the City and County of Denver and the Colorado Department of Public Health and Environment for the “Chronic Disease Framework” program and the funding therefor.
Amends a grant agreement with Colorado Department of Public Health and Environment to add $585,177.00 for a new total of $685,177.00 and to add one year for a new end date of 6-30-2027 to support the creation and implementation of Chronic Disease health assessment, framework and infrastructure, citywide (ENVHL-202582823/ENVHL-202582823-01). The last regularly scheduled Council meeting within the 30-day review period is on 8-31-2026. The Committee approved filing this item at its meeting on 7-29-2026.
