A resolution approving a proposed Amendatory Agreement between the City and County of Denver and The Colorado Coalition for the Homeless for housing placement and treatment services for adults experiencing chronic homelessness.
Amends a contract with The Colorado Coalition for the Homeless by adding $9,041,667 for a new total of $10,241,667 for housing placement and treatment services for adults experiencing chronic homelessness, citywide. No change to contract duration (HOST- 202159074-01). The last regularly scheduled Council meeting within the 30-day review period is on 11-15-21. The Committee approved filing this item at its meeting on 10-13-21.

Type and Scope of services to be performed:
Permanent Supportive Housing services will be provided to 180 individuals experiencing homeless per year. Additionally, case management and supportive services will be provided to 54 participants in the Tennant Based Rental Assistance program annually.
These services including the following:
A. Access to a multidisciplinary treatment including:
Crisis response, nursing care, psychiatry/medication support, treatment planning, case management, addiction assistance, care coordination, peer support, vocational services, individual therapy, group therapy, and life skills development for 60 individuals experiencing homelessness annually.
B. Housing stabilization:
Under the Housing First Expansion Subprogram, will provide access to treatment, housing stabilization services, bridge rental assistance and housing subsidy assistance, and linkage to community supports to about 120 Denver residents experiencing homelessness per year by providing the following: Crisis response, nursing care, psychiatry/medication support, treatment planning, case management, addiction assistance, care coordination, peer, support, vocational services, individual therapy, group therapy, and life skills development. As well as provide financial assistance related to housing costs such as but not limited to rental assistance.
C. Linkage to community supports:
Under the Transitional Housing Subprogram, CCH will provide case management, to about 54 participants per year and include the following services: home-based case management to families and individuals in the transitional housing program utilizing the HOME Investment Partnerships Programs (HOME) subsidies under the Tennant Based Rental Assistance program. Case Managers will also provide direct client supports including
transportation assistance, food assistance, legal assistance, and rental application fees.
• Housing Navigators will assist participants in identifying housing units in the community that fit participants’ needs. The Housing Navigator and team will work directly with private landlords to secure low-barrier access to safe and affordable housing units in the Denver community that accept rental subsidies.
• Housing Intake Coordinator will aid participants in gathering documents and necessary paperwork for acceptance into the HOME voucher subsidy program and will support clientele exiting the HOME program onto other subsidies after the transitional housing period is completed
