Type and Scope of services to be performed:

Colorado Coalition for the Homeless (CCH) will provide Emergency Shelter Activities and day-time integrated health services located within an emergency shelter located at 4600 E 48th Ave Denver, CO 80216 to people experiencing homelessness. These services work to prevent the spread of COVID-19 for people experiencing homelessness by providing health services on site in congregate shelter settings. This contract will serve 1300 unduplicated individuals.

 

Essential Services funded through this contract include:

1.                     Outpatient Health Services

2.                     Mental Health Services

3.                     Substance Abuse Treatment Services

4.                     Case Management Services
